Default Here's an idea to lower the cost of medicine,

get rid of the millions, and millions spent on public advertising...

Seriously, leave it to the sale reps to sell this shit to the doctors that prescribe it to their patients...

I can't make it through 6PM evening network news with my two kids without them hearing/seeing about erectile dysfunction or depression, or quitting smoking, all with the help from scripts... meds for sex this, and meds for sex that...

Maybe PBS wasn't so bad after all...

    WTF... Fucking over-medicated society...

I actually did my pharmacy management paper on this very thing. You know those commercials aren't directed at the prescriber right?

    Yep.

But if the drug companies didn't spend a billion dollars + on advertising to the public they'd pass that along to their actual customers.

you let your kids watch the 6 oclock news? Damn, that's the most corrupting stuff on TV right there! Better to have 'em watch some amputee porn!

you're absolutely right...the entire pharma industry is a mess and there are a ton of things that could be done to revamp it. The commercials are very very problematic.
    Stay out of it, dude.

we don't have nearly the advertising for meds up here - not sure what the reason it. Sure we get our sure of Viagara commercials but outside of that not much. Every time I'm in the states I'm amazed that it seems every 2nd commercial is for a cure for something.

Do you think that has to do with your universal healthcare system or because you guys simply aren't as pill happy as we seem to be?

i honestly don't know broken. I think more the pill happy part since we don't have pharmacare - u either have private insurance for that or it comes out of your pocket. I just find is almost comical when i'm watching an american network (like CNN) and you get 3 or 4 straight commercials for a pill. I'm thinking isn't it a doctor's job to prescribe medication? Do people actually go to their doctor and say I think I need "this pill"? If i ever felt the need to tell my doctor what i needed, i think i'd change doctors.

That's exactly what happens. Since people can choose their own doctor here, if that doc doesn't give it to them, they go down the street. It pressures them to give in to patient requests.

Here's a great video showcasing what private entrepreneurship in health care--without the involvement of insurance companies (except in catastrophic events)--could look like. I can't believe this was on PBS:

    http://www.pbs.org/newshour/globalhealth/

Private businesses do a service for a fee. This clinic is no different. In this case, the owner used his own money to start the business and didn't take a salary to get this eye clinic started. As the video mentioned, profit margin is small, so quantity and efficiency is important, unlike government. There's no profit in government-run programs, and it's not their money (it's ours), so there is no incentive for efficiency. By screening their patients, those who can pay do, and that not only allows for salaries for the employees--which have increased quite well according to the video--but also for pro bono work.

The market in India disallows the clinic from charging outrageous prices, and so does the fact that it's a cash-only business. When someone else pays, i.e. insurance companies, competition is eliminated, and the process of inflated costs begin. Doctors offices know people aren't shopping prices because no matter where you go you pay the same monthly payment to your insurance company, and all anyone is responsible for is the copay on each visit (with typical coverage). Not only does this drive up prices, but so does the increased demand for a finite product (medical clinics and doctors don't grow on trees) by those who visit a doctors office more often without a significant cost increase, or those who go to the doctor for any reason when if they had to pay out of pocket, or those that might otherwise stay home if they woke up coughing with a runny nose and didn't wanna pay cash out of pocket.

Of course, the increased demand drives up prices which in turn drives up insurance rates. If a consumer wants to spend less on insurance, the insurance company ends up telling you where you can and cannot go, and what they will and will not pay for--otherwise known as rationing.

High deductible, low monthly payment, catastrophic insurance would still be needed in case of emergencies, but much like your car insurance, which doesn't pay for oil changes, the consumer would be responsible for their own basic upkeep and maintenance, and insurance would kick in in the event of a heart attack, cancer, etc.

I like this video because it is the perfect argument against government provided health care. The real reform we need here is to embrace private companies like this. Good ideas in the marketplace will be successful, and the personal responsibility of consumers without the aid of insurance for basic health upkeep would drive prices down to what the market could bear. The problem is, places like this don't exist in a world where insurance pays for any medical visit no matter how small. I would be more likely to pay a bit bit more at a clinic like this, as opposed to a cash-only clinic that didn't do pro bono work, so that those who cannot afford the care could get it. However, that would be a personal choice, and those that wanted to spend a little less could do so. It's the same way that I can choose to pay more at SuperTarget or Vons for a better environment (or maybe because they give more to charity, etc.), or I can choose to pay less at SuperWalMart when I'm pinching pennies at the end of the month.

And yes, of course, loser pays with lawsuits would be the other key to change.

we choose our own doctors up here too but i haven't heard about patients pressuring them into giving them a certain drug against the judgement of the doctor. Maybe it happens and I haven't heard about it, not sure.

well...doctor's do get bonuses/spiffs/extra funding/financial incentives to prescribe various drugs.

Has anyone ever walked into a doctor's office and seen signs and posters with a certain drug's logo and then borrowed a pen and it was that logo and then, what do you know, the doctor thinks you need to have a script for that drug?

There are doctors whose practices are underwritten by pharmaceutical companies...kinda like little league teams or old fashioned soap operas. It's live product placement. I'm fortunate in that my doctor won't even see the sales reps. They come by and drop off samples with his nurses and he gives them away instead of writing scripts. There have been times I've walked out with a month's supply worth of samples of something and then instead of calling in a refill, I swing by and pick up more samples. He refuses to play the game. Over the past couple of years I've noticed that there are less and less samples because the reps are wise to the fact that he won't sign contracts with the companies.

That in and of itself is a HUGE conflict of interests.
    Stay out of it, dude.
