Might've been something that slipped by the artwork people - on the back cover and on the inner sleeve by his picture, it says "David Roth", however on the label and on the sleeve where it says "All selections written by..." it has "David Lee Roth"

Maybe it's just because I'm so used to it.. but David Lee Roth flows much better than David Roth. And it sounds less.. how should I put this.. common??

It's "David Lee Roth" for show business reasons. It's along the same lines as people using stage names (i.e. Nikki Sixx) and actors feeling compelled to be known by their middles names or initials also (i.e. Michael J. Fox.) It sounds different and since it sounds different it further separates these people from Joe Public. DLR was big on the show business kick. Another example was when he had the idea to tell everyone the members of the band were 2 years younger than they actually were.

I agree that David Lee Roth is much more cool then David Roth...

The answer may even be more involved then that, especially for actors (which may not include the Diamond One, but it is an interesting factoid). Actors and anyone involved in television and screen productions generally must register for the Screen Actors Guild (or something like that, I may have the name wrong)...but if someone else had already registered with the name "Michael Fox" then even if your name is Michael Fox, you can't use that name. It must be something different, which ended up being "Michael J. Fox." If that name is already registered and you have the exact same name, and you want to register then you must use your middle name or come up with something else.

I don't know if this applies to musicians or not, in fact I doubt it. I think Dave picked up the Lee because it sounded much less common...
